Photography & Media Consent Form - Editable Template for Aesthetic Clinics

Sale price$15.00 AUD

The consent form that separates your clinical records from your marketing — before it becomes a problem.

Most clinics collect photo permission once, in a single line buried at the bottom of an intake form, and then use those images for everything. That is the version that gets you into trouble. This form does it properly: clinical photography is handled as part of the health record, and every marketing use is a separate, optional, individually initialled permission the client can decline or withdraw.

What's inside

  • Clinical photography section: explains why before-and-after images form part of the client's record, and how they're stored and protected.
  • Five separate marketing consents: internal training, professional education, website and print, social media, and paid advertising. Each ticked and initialled individually, because consent to a training slide is not consent to an Instagram ad.
  • How images may be shown: face in full, cropped to the treated area, eyes obscured, first name used. Four questions that prevent the most common complaint a clinic receives.
  • What the client should understand: plain-language explanation that published images can be copied and shared, that nothing will be filtered or retouched to misrepresent a result, and how to withdraw consent.
  • Signature and witness block.
